Creates the Nebraska Medical Cannabis Commission (three members of the Nebraska Liquor Control Commission serve ex officio, with up to two additional gubernatorial appointees) to license and regulate cultivators, dispensaries, products manufacturers, testing facilities, and transporters of medical cannabis

Establishes a patient registry program with $45 enrollment/renewal fees, requiring a written recommendation from a health care practitioner for any qualifying medical condition where the practitioner determines cannabis benefits outweigh potential harms

Limits licenses until January 1, 2030: five vertical licenses (allowing four dispensaries, one products manufacturer, one cultivation license each), plus ten dispensary, five cultivator, five products manufacturer, and five testing facility stand-alone licenses statewide

Imposes a special 4% sales tax rate on medical cannabis sales by dispensaries, with proceeds credited to the School District Property Tax Relief Credit Fund

Prohibits possession of open cannabis containers in motor vehicles, applies the Nebraska Clean Indoor Air Act to cannabis smoking, and removes medical cannabis from the marijuana and controlled substances tax when lawfully possessed
