Applies to sickness and accident insurance policies, subscriber contracts, and hospital/medical/surgical expense-incurred policies delivered, issued, or renewed in the state

Extends coverage requirements to self-funded employee benefit plans to the extent not preempted by federal law

Excludes policies that provide coverage only for specified diseases or other limited-benefit coverage

Allows insurers to continue applying deductibles, copayments, and network incentives contained in their policies, contracts, or plans
