Creates an early support system in all Nebraska public school districts to identify students showing two or more warning indicators including chronic attendance changes, declining academics, behavior challenges, lack of engagement, hygiene concerns, or exposure to adverse childhood experiences

Requires each school district to convene a youth support coordination team that may include counselors, teachers, community health workers, behavioral health liaisons, and family advocates to conduct trauma-informed assessments and coordinate referrals

Mandates inclusion of community health workers on coordination teams through partnerships with federally qualified health centers, local public health departments, and behavioral health organizations

Prohibits referrals to law enforcement or child welfare based solely on early support system indicators unless legally required due to imminent risk, with DHHS monitoring to prevent disproportionate impact based on race, disability, income, or language

Requires annual implementation reports from school districts to the State Department of Education, which must compile and submit a report to the Legislature evaluating students identified, interventions provided, and outcomes related to attendance and achievement
