Requires the Department of Health and Human Services to submit a Section 1115 reentry demonstration waiver to CMS by July 1, 2025, providing Medicaid coverage for incarcerated individuals transitioning back to society, including case management, medication-assisted treatment, a 30-day supply of prescriptions upon release, and housing supports

Requires a separate Section 1115 demonstration waiver for health-related social needs by July 1, 2025, covering rent or temporary housing for up to six months, food assistance for up to six months, and case management for formerly incarcerated individuals

Provides a subsidy of up to $100 per individual for internet-connected devices such as mobile phones or laptop computers for formerly incarcerated individuals

Covers infrastructure investments for correctional facilities and social services, including technology, operational practices, workforce development, and marketing outreach

Amends Section 68-911 of the Revised Statutes Cumulative Supplement, 2024, which governs medical assistance program coverage requirements
