Requires transition plans within 60 days of admission and employment planning within 30 days of intake, with aftercare coordinators assigned to each youth providing up to 12 months of post-release support

Mandates transition plans include housing stabilization, school reentry, behavioral health continuity, identification documents, family engagement, and joint meetings with probation officers and case managers every 30 days for the first 90 days

Utilizes credible messenger mentoring (individuals with lived experience in justice or foster care systems) and caps caseloads at 15 youth per aftercare coordinator

Requires implementation through existing appropriations with no new General Fund spending; becomes operative July 1, 2026, with emergency clause for immediate effect upon passage
