Health care providers must furnish patients with a written or electronic statement listing all medical services billed to their insurance carrier within 15 business days of claim submission

Required statement information includes: service description, date rendered, billing codes used, amount billed to insurance, amount paid or denied by insurance, and any remaining patient balance

Disclosure may be provided electronically via secure patient portals, email with patient consent, or in hard copy upon request

Applies to all licensed or certified health care providers including hospitals, clinics, physicians, and other medical professionals

Takes effect 60 days after passage
