Allows parents or legal guardians to submit any written signed statement attesting to a religious objection to immunization, rather than requiring use of a specific department-provided form

Imposes civil penalties of up to $1,000 per violation on DHHS officers, employees, or agents who distribute vaccination promotional material without the required exemption notice

Mandates DHHS submit an annual compliance report by December 31 to legislative health committees listing all promotional campaigns, whether the required notice was included, and any violations or penalties assessed

Estimated General Fund expenditures of $43,000 in FY27, $45,500 in FY28, and $47,500 in FY29 for a part-time position to implement requirements and produce the annual report
