Health carriers must provide participating providers or facilities with 60 days notice before making contract changes, including changes to attachments, exhibits, and provider manuals

Contract changes are limited to 4 times per calendar year, effective only on January 1, April 1, July 1, or October 1, with exceptions for mutual agreement, state/federal government requirements, or AMA procedural terminology code changes

Carriers must provide both original and revised versions of contract documents, with additions shown in bold underlined text and deletions shown with strikethrough formatting

Contract changes resulting in aggregate reimbursement impacts exceeding $500,000 per calendar year across all participating providers in the state require carriers to submit good faith financial impact estimates to each affected provider

The Insurance Commissioner must publish annual estimates and explanations of financial impacts from qualifying contract changes on the department's official website or make them publicly available upon request
