Health carriers must pay nonparticipating ambulance providers the lesser of: the county/municipality rate, 325% of the Medicare rate for the same service and geographic area, or the provider's billed charges

Prohibits balance billing by nonparticipating ambulance providers—once the health carrier pays according to the formula, patients cannot be billed for additional amounts beyond their normal cost-sharing

Patient cost-sharing (copays, coinsurance, deductibles) for out-of-network ambulance services cannot exceed what would be charged for in-network ambulance services

Health carriers must pay clean claims directly to nonparticipating ambulance providers within 30 days; payments cannot be sent to the patient

Effective date is January 1, 2026; fiscal note estimates approximately $15 million in aggregate premium increases (about 0.5% or $1.13-$1.38 per member per month) in the commercial insurance market
