Applies to infants born on or after the bill's effective date; children born before January 1, 1998 may still be enrolled only upon written parental request

Requires written consent from the individual (or parent/guardian for those under 18) for any person to be enrolled in the immunization registry, regardless of other laws or regulations

Maintains existing provisions allowing parents, guardians, or adults over 18 to remove or inactivate their information from the registry at any time

Preserves the prohibition against healthcare providers discriminating against individuals who choose not to participate in the registry
