Lowers the age for baseline mammogram coverage from 40 to 35 years old, with annual mammograms required for women 35 and over

Women under 35 with family history of breast cancer or other risk factors must receive coverage for mammograms at medically necessary intervals determined by their health care provider

Requires coverage for additional testing (ultrasound, MRI, 3D mammography) after baseline mammograms showing extremely dense breast tissue, abnormal results, or when patients have elevated risk factors

Applies to all health insurers including hospital/medical service corporations, commercial insurers, HMOs, individual and small employer health benefits plans, and the State Health Benefits Program (SHBP)

Adds new requirement for School Employees' Health Benefits Program (SEHBP) to provide the same mammogram coverage, which was not previously mandated under state law
