Eliminates religious exemptions from mandatory immunization requirements for children attending child care centers, preschools, elementary and secondary schools, and higher education institutions in New Jersey

Establishes two permitted exemption types: medical exemptions requiring a licensed physician, osteopath, or advanced practice nurse to certify a vaccine is medically contraindicated consistent with federal ACIP guidelines, and sibling vaccine injury exemptions for children whose siblings received a final vaccine injury determination from the federal Vaccine Injury Compensation Program or a court

Allows nonpublic (private) child care centers and schools to adopt policies admitting unvaccinated children who don't qualify for exemptions, provided they post immunization rates at entrances, require signed acknowledgement forms, and reserve the right to exclude unvaccinated children during disease outbreaks

Requires approved medical and sibling vaccine injury exemptions to be entered into the New Jersey Immunization Information System, with the registry to be updated within one year to accommodate this information

Takes effect 180 days after enactment and authorizes county and local boards of health to audit approved exemptions
