Amends the New Jersey Immunization Information System (NJIIS) to automatically enroll any person not currently in the registry when they receive a vaccine, unless they submit a written opt-out request

Maintains existing enrollment rules: newborns born on or after January 1, 1998 are automatically enrolled at birth unless parents opt out; those born before that date may enroll by written request

Allows parents or legal guardians to opt out on behalf of minors under 18 years of age through written request

Prohibits health care providers from discriminating against individuals who choose not to participate in the registry

The NJIIS serves as a centralized immunization record repository accessible to health care providers, schools, colleges, licensed child care centers, and authorized public and private organizations
