Expands the definition of "participating county" to include counties with a municipality having population greater than 30,000 and a 2020 Municipal Revitalization Index Distress score of 33 or higher, based on 2019 ACS 5 Year Survey Data

Excludes from the expanded definition counties with a municipality over 125,000 population, counties under 150,000 total population, and counties with median household income exceeding $110,000

Caps the local healthcare-related fee that participating counties may impose on hospitals at the federal maximum aggregate amount minus one percent of total net patient revenues

Requires hospitals to reimburse the State for any federal funds the State must repay if hospital payments exceed federal disproportionate share limits

Shields participating counties and Medicaid Managed Care Organizations from liability for expenditure plan amounts later determined impermissible by federal agencies
