Coverage for PGT with IVF may be limited to cases where both partners are known carriers of an autosomal recessive disorder, one partner carries a single gene autosomal or X-linked disorder, or the partners already have a child diagnosed with the genetic condition

Genetic conditions must result in significant health problems or severe disability if passed to offspring to qualify for mandatory coverage

Applies to group health insurance policies covering more than 50 persons, including hospital service corporations, medical service corporations, health maintenance organizations, and State/School Employees Health Benefits Programs

Religious employers may request exclusions from IVF-related coverage requirements if contrary to their bona fide religious tenets, with insurers required to provide written notice of such exclusions to subscribers
