Prohibits New Jersey marriage or civil union license forms from requiring applicants to disclose the number of times they have previously been married or entered into a civil union

Eliminates the requirement for applicants to provide the name of their most recent spouse or civil union partner on the license form

Retains existing requirements for applicants to provide name, age, parentage, birthplace, residence, Social Security number, domestic status, and parents' names and county of birth

Social Security numbers on the forms remain confidential and can only be released for child support enforcement purposes

Takes effect immediately upon enactment
