Requires licensed health care facilities and programs serving children to perform Lyme disease screening during well-child visits using a standardized checklist developed by the Department of Health

Mandates health care providers administer the Lyme screening checklist during well-child visits and symptom-related visits (fever, fatigue, rash, joint pain, headache) occurring between April 1 and October 31 each year

Requires providers to document screening responses in medical records and order Lyme testing or refer patients for further evaluation when results indicate potential risk

Requires the State Board of Medical Examiners to mandate one credit of continuing medical education on Lyme disease for physician license renewal, covering prevention, treatment, pediatric health risks, and communication with parents

Takes effect on the first day of the sixth month following enactment
