Establishes the "Continued Access in Underserved Communities Pilot Program" allowing hospitals relocating from underserved communities to maintain a "community access location" at their original site with services including 48 medical/surgical beds, emergency department, psychiatric services, surgical services, outpatient clinics, and diagnostic imaging

Defines "underserved community" as a municipality with over 30,000 residents, only one hospital, located in a Medically Underserved Area with an Index of Medical Underservice Score of 62.0 or less, and identified as underserved by the Commissioner of Health

Participating hospitals must relocate to property that was federally owned on or before December 31, 2005, and the community access location will operate under the same license as the relocated hospital

Appropriates $3 million from the General Fund to provide financial assistance to eligible hospitals for developing and maintaining community access location services, infrastructure, and community health programs

Pilot program expires 10 years after the participating hospital obtains a certificate of occupancy for the relocated facility or when appropriations are no longer available, though psychiatric services may continue at the original location after expiration
