Requires health care practitioners to provide patients (or parents/guardians of minors) with the vaccine manufacturer's insert, CDC vaccine excipient summary, and VAERS reporting information at least 48 hours before administering any vaccine.

Mandates that patients or their guardians must sign and return a copy of the vaccine insert confirming consent before the vaccine can be administered.

Requires practitioners to offer patients the option to opt out of vaccination, along with a description of potential implications including disease transmission risks and possible school attendance restrictions.

Prohibits health care practitioners from refusing to provide services to a patient or transferring their care solely because the patient declined a vaccine.

Takes effect 30 days after enactment; pre-filed for introduction in New Jersey's 2026 legislative session.
