Requires all health insurance carriers in New Jersey to cover male sterilization procedures, regardless of whether the plan includes outpatient prescription drug coverage

Eliminates all cost-sharing (deductibles, coinsurance, copayments) for prescription female contraceptives and male sterilization procedures under covered health plans

For high-deductible health plans, the no-cost-sharing requirement applies only after the enrollee meets the minimum annual deductible amounts specified under federal IRS rules for Health Savings Account-eligible plans

Applies to hospital service corporations, medical service corporations, health service corporations, HMOs, individual and group health insurers, small employer health plans, prepaid prescription organizations, and State Health Benefits Commission contracts

Takes effect 90 days after enactment and applies to policies issued or renewed after the effective date; maintains existing religious employer exemptions for organizations that are churches or church-controlled schools
