Establishes the Farm Vitality Planning Reimbursement Grant Program through the Department of Agriculture to reimburse farmers for costs incurred in developing and implementing long-term farm viability plans

Authorizes grants of up to $7,500 per eligible plan, covering up to 75% of total eligible costs, with farmers responsible for financing the remaining costs through monetary means

Eligible plans include business plans for economic stability, agricultural diversification or expansion strategies, farm ownership transition plans (both family and non-family transfers), and preserved farmland maintenance plans

Reimbursable costs cover professional services including accountants, business consultants, appraisers, attorneys, facilitators, and mediators; excludes capital improvements, conservation plans, taxes, insurance, and travel expenses

Requires the Secretary of Agriculture to submit annual reports to the Governor and Legislature evaluating the program's effectiveness, number and dollar amount of grants issued, and recommendations for continuation or additional funding
