Creates a 27-member New Jersey Commission on Health Equity within the Department of Health, including legislators, heads of 16 state departments or their designees, and a local health department representative serving a 4-year term.

Requires the commission to examine how social determinants—including housing, education, employment, transportation, environmental factors, food insecurity, and criminal justice—affect health outcomes and disparities among racial, ethnic, and socioeconomic groups.

Mandates the commission develop a statewide health equity plan, make recommendations on implicit bias training for clinicians, and advise on enforcement of National Standards for Culturally and Linguistically Appropriate Services (CLAS Standards).

Establishes a Health Equity Data Advisory Committee in coordination with the New Jersey Health Information Network to define data collection parameters and maintain a health equity data set from healthcare facilities and insurers.

Requires the commission to submit an initial report to the Governor and Legislature within 18 months of enactment, with annual reports thereafter on findings and legislative recommendations.
