Requires health care professionals who screen, diagnose, or treat patients with amyotrophic lateral sclerosis (ALS) or motor neuron disease to confidentially report all cases to the New Jersey State Health Assessment Data System within six months of diagnosis

Motor neuron diseases covered include ALS, progressive bulbar palsy, primary lateral sclerosis, progressive muscular atrophy, spinal muscular atrophy, Kennedy's disease, and post-polio syndrome

Authorizes the Department of Health to share reported data with the CDC, local health officers, and researchers studying ALS or motor neuron disease for public health and research purposes, with protections against disclosure of personal identifying information

Requires the Commissioner of Health to develop and publish educational materials on the department's website within 12 months about how to access ALS and motor neuron disease datasets and statistics

Data collected must be used for research and statistical purposes only, with all reporting designed to protect patient and health care professional confidentiality
