Health insurance carriers must cover diagnostic breast examinations (to evaluate abnormalities found during screening or other exams) without any deductibles, copayments, or coinsurance

Supplemental breast examinations for individuals at increased risk of breast cancer must also be covered without cost-sharing, including those with dense breast tissue or relevant family/personal medical history

Covered examination methods include contrast-enhanced mammography, diagnostic mammography, breast MRI, breast ultrasound, and molecular breast imaging, following National Comprehensive Cancer Network Guidelines

Requirements apply to all major insurance types: commercial insurers, HMOs, hospital/medical/health service corporations, individual and small employer plans, and State/School Employees' Health Benefits Programs

Takes effect on the first day of the fourth month following enactment and applies to all new and renewed policies after that date
