Applies to both new credentialing applications and credential renewals for physicians seeking to participate in carrier provider networks

Covers physicians employed by hospitals or other health care facilities that seek to participate in carrier networks

Carriers may still request additional information from applicants beyond the universal application form, provided it does not duplicate existing form information

Takes effect immediately upon enactment and applies to applications received on or after the effective date
