Authorizes the NJ Motor Vehicle Commission to issue special New Jersey 4-H license plates with a design and emblem selected in consultation with New Jersey 4-H

Requires a $50 application fee plus $10 annual renewal fee, with proceeds deposited into the "New Jersey 4-H License Plate Fund" to support the organization's mission and programs

Prohibits use of state or public funds for initial implementation costs; New Jersey 4-H or designee must contribute up to $25,000 in non-public monies to offset startup costs

Program will not launch until New Jersey 4-H provides required startup funds and submits at least 500 completed license plate applications

Chief administrator may discontinue the program if the average cost per plate exceeds $50 for two consecutive fiscal years; bill expires if the 500-application threshold is not met within 12 months of enactment
