Required information includes detailed descriptions of programs such as the Supports Program (Section 1115 waiver), Community Care Waiver, and self-directed services program

Eligibility criteria for each program must be specified, including Medicaid requirements, level of care assessments, and funding amounts based on individual needs

List must include information about transitional planning services and guardianship services available through the division

Division must keep the list updated and prominently display information about webinars and presentations that help individuals access services; takes effect first day of fourth month after enactment
