Creates special "Mental Health Awareness" license plates for New Jersey vehicles, with design selected by the Motor Vehicle Commission Chief Administrator in consultation with the Division of Mental Health and Addiction Services

Requires a $50 initial application fee plus $10 annual renewal fee, in addition to standard registration fees, with proceeds deposited into a new "Mental Health Awareness License Plate Fund"

Fund revenues will be appropriated annually to the Division of Mental Health and Addiction Services to conduct mental health research and support mental health programs and services throughout New Jersey

Prohibits use of state or public funds for initial program costs; requires a designated individual or entity to contribute up to $25,000 and submit at least 500 completed applications before the program can begin

Program may be discontinued if the average cost per license plate exceeds $50 for two consecutive fiscal years, and the bill expires if startup requirements are not met within 12 months of enactment
