Prohibits New Jersey marriage and civil union license forms from requiring applicants to disclose the number of times they have been previously married or entered into a civil union

Removes the requirement for applicants to provide the name of their most recent spouse or civil union partner on license applications

Amends R.S.37:1-17, which governs the information collected on marriage and civil union licenses by the New Jersey Department of Health

Retains existing requirements for applicants to provide name, age, parentage, birthplace, residence, Social Security number, domestic status, and parents' information

Takes effect immediately upon enactment
