Requires physicians and medical examiners to complete the federal CDC Sudden Unexplained Infant Death Investigation Reporting Form (SUIDI form) for any death of a child under three years old where the cause is not immediately obvious.

Mandates that the infant's physician attach a record of all vaccines administered in the six months prior to death as an addendum to the SUIDI form, which must be submitted to the Child Fatality and Near Fatality Review Board within 10 days.

Expands the duties of New Jersey's Child Fatality and Near Fatality Review Board to identify and investigate infant deaths and near-deaths that may have resulted from vaccination.

Requires the board to designate at least one regional team specifically to review vaccination-related infant fatalities and near fatalities.

Mandates the board submit a report to the Governor and Legislature within one year, with supplemental reports at least every five years, detailing vaccination-related infant death rates, patterns, and recommendations for reducing such fatalities while maintaining public health protections.
