Requires written consent with specific disclosures before sharing protected information, including the exact record being shared, purpose, recipient, and statements that consent is voluntary and may be revoked

Makes violations subject to injunctive relief and civil liability for damages; government entities must provide 90-day notice to affected individuals when compelled to disclose protected information pursuant to judicial orders or federal law
