Requires the Division of Medical Assistance and Health Services to establish a mandatory incentive-based value payment system for home health agencies and health care service firms serving NJ FamilyCare recipients enrolled in Fully Integrated Dual Eligible Special Needs Plans.

Performance rankings will be based on nine "Outcome and Assessment Information Set" measures including acute care hospitalization rates, emergency care incidence, patient bathing, ambulation, transfers, provider collaboration, medication management, surgical wound status, and adverse events.

Appropriates up to $10 million for the program, with 60% distributed to agencies ranked in the top 20% for absolute performance and 40% to those in the top 20% for improvement relative to baseline.

Creates an eight-member Home Health Services Incentive-Based Value Payment System Advisory Board appointed by the Governor to provide regulatory recommendations within six months, with regulations due within one year of enactment.

Requires annual independent audits beginning two years after enactment, annual legislative reports on system status and outcomes, and data sharing with Rutgers Center for State Health Policy for program evaluation.
