Authorizes health care professionals to provide preventative, primary, specialty, or urgent care in the homes of disabled individuals or elderly persons age 65 or older with mobility issues who are unable to visit a health care facility.

Requires all health insurance carriers in New Jersey—including insurance companies, health service corporations, HMOs, and State Health Benefits Program administrators—to cover home care expenses at the same level as other medical conditions.

Defines "disabled" as total and permanent inability to engage in substantial gainful activity due to a medically determinable physical or mental impairment, including blindness.

Home care eligibility requires documentation by a health care professional in the patient's medical record based on good faith clinical judgment that the individual cannot present for in-person visits.

Explicitly excludes custodial care from the definition of covered home care services.
