Division of Aging Services must create a single unified application form for all federal and State-funded programs it administers, serving adults aged 65 and older or those aged 18-64 receiving Social Security Disability

Applications will be automatically processed for enrollment in all eligible programs administered by the division, unless the applicant opts out or is determined ineligible

The unified form must collect all information necessary to determine eligibility across all division programs

Takes effect on the first day of the third month following enactment, with the Department of Human Services authorized to take preparatory administrative actions beforehand
