Allows a surviving spouse to revert their driver's license name back to their birth name after their spouse dies by presenting their marriage certificate and spouse's death certificate to the Motor Vehicle Commission

Requires the licensee to attest they have not been convicted of a crime, have no pending criminal charges, and are not seeking the name change to avoid prosecution, evade creditors, or commit fraud

Amends P.L.1987, c.20 to add this death certificate provision as an alternative to the existing name change requirements (marriage, divorce, or court order)

Updates statutory language to replace "director" with "chief administrator" and "New Jersey Merit Rating Plan" with "Motor Vehicle Violations Surcharge System"
