Establishes a three-year pilot program requiring NJ FamilyCare (Medicaid) to cover food prescriptions for enrollees diagnosed with Type II diabetes, hypertension, or obesity when written by a licensed health care provider

Covered foods include fresh fruits, fresh vegetables, culturally-appropriate staple foods, and other medically-appropriate grocery items, with priority given to partnerships with local farmers and food suppliers

Expands the existing "Healthy Small Food Retailer Act" to include retail pharmacies, allowing them to participate in selling healthy foods and fill food prescriptions for NJ FamilyCare enrollees

Increases financial assistance available to participating small food retailers and retail pharmacies from $5,000 to $6,000 for refrigeration, display shelving, storage, and transportation equipment

Appropriates $500,000 from the General Fund to the Department of Human Services for implementation, with annual reporting requirements on participation rates, health outcomes, costs, and potential Medicaid savings
