Prohibits general acute care hospitals and clinical laboratories from collecting or requesting sexual orientation or gender identity data from patients under age 18, unless the patient, parent, or guardian voluntarily provides it.

Any voluntarily provided sexual orientation or gender identity data for minors may only be used for medical diagnosis and treatment purposes, not for any other purpose.

Requires electronic medical records and laboratory information management systems to be configured so that gender identity and sexual orientation fields are not mandatory for saving demographic information when the patient is under 18.

Amends P.L.2021, c.454, which currently requires hospitals and labs to collect sexual orientation and gender identity data from all patients and mandates these fields for electronic record systems.

Directs the Commissioner of Health to adopt rules and regulations to implement these changes, with the act taking effect immediately upon passage.
