Creates special Mental Health Awareness license plates in New Jersey, displaying the words "Mental Health Awareness" with a design selected by the Motor Vehicle Commission in consultation with the Division of Mental Health and Addiction Services

Requires a $50 application fee plus a $10 annual renewal fee, in addition to standard registration fees, with proceeds deposited into the Mental Health Awareness License Plate Fund

Fund monies will be appropriated annually to the Division of Mental Health and Addiction Services to conduct mental health research and support new and existing mental health programs throughout New Jersey

Prohibits use of state or public funds for initial program costs; requires a designated entity to contribute up to $25,000 in non-public money and submit at least 500 completed applications before the commission can begin producing plates

Program may be discontinued if the average cost per plate exceeds $50 for two consecutive fiscal years; bill expires if funding and application requirements are not met within 12 months of enactment
