Creates a special enrollment period for individual health insurance enrollees when their health care provider or a dependent's provider leaves the plan's network during the current coverage period.

Requires the New Jersey Individual Health Coverage Program board to establish this special enrollment period, with a minimum duration of 30 calendar days.

Mandates that insurance carriers notify affected enrollees about the special enrollment period, including start and end dates.

Takes effect 90 days after enactment and applies to health plans delivered, issued, executed, or renewed on or after that date.
