New Jersey Department of Education must develop and distribute a standardized transition checklist to help students with disabilities prepare for post-secondary life, covering education, employment, healthcare, legal status, Social Security, Medicaid, and independent living.

IEP teams must review the transition checklist with students and parents at least twice per year, beginning the school year the student turns 16 (or younger if appropriate).

The checklist must include timelines and current contact information for agencies and community resources available to assist with the transition to adult life.

DOE must maintain and periodically update a website with transition resources providing easily accessible information on post-secondary opportunities and supports for students with disabilities, parents, and school districts.

The bill takes effect immediately upon enactment.
