Creates "Mission Critical Long-Term Care Teams" within the New Jersey Department of Health to identify and intervene at long-term care facilities experiencing operational or financial distress that may threaten resident health, safety, or continuity of care.

Each team will consist of four individuals with expertise in long-term care administration, financial management, nursing care, infection prevention, social work, quality improvement, safety, and continuing professional education.

Requires the Commissioner of Health to establish specific indicators and thresholds for evaluating facilities' operational and financial soundness, as well as their capacity to protect resident rights and meet licensing standards.

Teams may be dispatched when surveys, inspections, or complaints identify at-risk facilities, or at the request of facility management, to evaluate conditions and recommend corrective measures.

Facilities receiving a Mission Critical Long-Term Care Team must cooperate fully and provide access to all physical locations and all financial, operational, and programmatic information requested.
