Authorizes the Department of Human Services to impose financial penalties up to $25,000 per offense on developmental disability service providers for violations including operating without a license, employing persons on abuse registries, failing background checks, and substantiated findings of abuse, neglect, or exploitation

Creates the "Residential Facility Quality of Care Improvement Fund" as a nonlapsing revolving fund to collect all fines and penalties, which will finance quality improvement initiatives and administrative actions

Establishes tiered enforcement for unlicensed operations: written warning for first offense, up to $10,000 penalty for second offense, and up to $10,000 plus potential payment suspension for third and subsequent offenses

Requires providers to complete internal investigations of abuse, neglect, or exploitation allegations within 180 days and submit detailed reports including interviews, evidence summaries, findings, and guardian notifications, with penalties up to $10,000 for noncompliance

Allows the department to assign Quality Management Teams to chronically noncompliant providers and impose penalties up to $5,000 every three months until improvement is demonstrated
