Electronic medical record and laboratory information management systems used or sold in New Jersey must be configured to require entry of patient race, ethnicity, sexual orientation, and gender identity before saving demographic information, with vendors facing civil penalties up to $1,000 per day of noncompliance.

Clinical laboratories must electronically record race, ethnicity, sexual orientation, and gender identity for patients presenting with non-electronic test orders at patient service centers, and include this data in mandatory state health reporting.

General acute care hospitals and clinical laboratories must implement evidence-based cultural competency training programs for staff who collect demographic information from patients, with biennial refresher courses required.

Demographic selections must include specific categories: 12 race/ethnicity options (including "does not wish to disclose"), 6 sexual orientation options, and 7 gender identity options, with the Commissioner of Health authorized to modify these categories.

Patients cannot be compelled to disclose their race, ethnicity, sexual orientation, or gender identity; the bill takes effect 180 days after enactment, with cultural competency training requirements effective at 120 days.
