Requires the New Jersey Commissioner of Health to develop a voluntary maternity care experience survey covering pre-conception, pregnancy, childbirth, and postpartum periods.

Surveys must be distributed to women at hospitals, ambulatory care facilities, and birthing centers between 14 and 90 days after pregnancy termination or conclusion of reproductive care treatment.

Survey questions must address patient experiences with providers, responsiveness, availability, and whether key health information was offered, with optional demographic and socioeconomic data collection.

Collected data will be analyzed to identify statewide trends and disparities in maternity care across racial, cultural, and socioeconomic groups.

Commissioner must publish survey results online and submit a legislative report with recommendations within 18 months; the act takes effect 180 days after enactment.
