Mandates New Jersey Medicaid coverage for continuous glucose monitors (CGMs) and related supplies for individuals diagnosed with diabetes who are treated with insulin or have documented problematic hypoglycemia (glucose below 54 mg/dL)

Requires CGM recipients to have physician determination of sufficient training on the device and prescriptions consistent with FDA-approved indications for use

Establishes follow-up care requirements for continued coverage: visits with a treating practitioner every six months for the first 18 months, then annually thereafter

Directs the Commissioner of Human Services to apply for necessary federal Medicaid waivers or state plan amendments to implement the coverage and secure federal financial participation

Authorizes immediate adoption of implementing regulations, effective for up to six months pending formal rulemaking procedures
