Requires clear explanations on the first page for why claims were paid in full, paid partially, or denied, along with instructions for any required actions or available options for the covered person

Specifies that required information must appear only on the first page, use 12-point font, and be written in simple, plain language consistent with New Jersey's existing "Life and Health Insurance Policy Language Simplification Act"

Takes effect 90 days after enactment
