Requires public and private health insurance policies, health plans, and Medicaid to cover medically necessary treatment for severe obesity, including bariatric surgery for adults 18 and older and related preoperative/postoperative services such as psychological screening, counseling, behavior modification, physical therapy, and nutritional education

Defines "severe obesity" as a body mass index (BMI) of 40 or higher, or BMI of 35 or higher with an associated comorbidity such as hypertension, cardiopulmonary conditions, sleep apnea, or diabetes

Allows insurers to require a preoperative period of up to 3 months, require surgery be performed at facilities with Metabolic and Bariatric Surgery Accreditation from the American College of Surgeons, limit coverage to one bariatric surgery per lifetime, and require physician certification of medical necessity

Explicitly excludes coverage for injectable glucose-lowering drugs and other weight loss medications from the mandate

Takes effect January 1, 2026, applies to policies delivered, issued, or renewed on or after that date, and contains an unfunded mandate exemption for local government expenses related to the act
