Authorizes phased implementation with initial priority given to areas with high concentrations of elderly persons and limited access to long-term care planning resources, including county-based pilot programs

Requires specific outreach to demographic segments including non-English speakers, veterans, and rural residents to ensure inclusivity and accessibility

Requires annual reports to the Legislature by February 1 each year and development of a framework to monitor and evaluate program effectiveness
