Caps out-of-pocket costs for prescription insulin at $35 maximum for a 30-day supply, regardless of insulin type or amount prescribed

Applies to private health insurers including individual policies, group policies, health benefit plans, HMOs, fraternal benefit societies, hospital/medical service corporations, and managed care organizations

Covers insulin used to treat type I, type II, and gestational diabetes

Exempts Medicaid recipients, Children's Health Insurance Program participants, Public Employees' Benefits Program members, and local government employees from the $35 cap requirement

Takes effect October 1, 2025 for new policies, and applies to renewals or extensions of existing policies after that date
