Transfers authority to establish and administer the Program of All-Inclusive Care for the Elderly (PACE) from the Aging and Disability Services Division to the Department of Health and Human Services, which must establish such a program serving Medicare and Medicaid recipients age 55 and older.

Allows the Department to contract with public or private agencies, including federally-qualified health centers, to administer PACE services, with contracts specifying service types, capitation rates, and evaluation criteria.

Requires the Department to apply for necessary federal waivers or State Medicaid Plan amendments and adopt regulations to implement the program.

Appropriates $293,055 for FY 2025-2026 and $184,057 for FY 2026-2027 from the State General Fund, plus authorizes additional non-General Fund expenditures of $323,855 and $91,953 respectively, to the Division of Health Care Financing and Policy for program costs.

Takes effect January 1, 2026 for most provisions, with appropriations effective July 1, 2025 and regulatory preparation authorized upon passage.
